우선 가고자 하는 곳에 편지를 띄우는데 대개 2년전에서
1년 6개월전에 시작을 한다. 같은 편지를 여러 곳에 한꺼번에 띄우는
것이 시간절약을 위해 좋은 방법일 것으로 생각한다.

It's a great pleasure to have a chance
to write to you. I have come to know of you through your worthy
articles. I am writing this letter to inquire about research fellowship
opportunity in Obstetrics and gynecology Research Laboratory.
I am a 35-year-old male physician in the Department of Obstetrics
and Gynecology, Chungbuk National University Hospital and also appointed
as Assistant Professor of Obstetrics and Gynecology, Chungbuk National
University College of Medicine. I graduated from Seoul National
University College of Medicine in 1986 and have been working here
for three years after finishing my residency at Seoul National University
Hospital which is the top-ranked in Korea as you know.
Working as a clinician, I did not have enough chances to be involved
in basic research works. Thus, I decided to look for a further training
position such as a research fellowship to broaden my medical experience
and knowledge. I am interested in the research fields of pathophysiology
of preeclampsia. If you have any research fellowship programs or
any other opportunities for foreign medical doctors such as myself,
would you please send me some details. Although my school usually
provides a small part of the required stipend, it would be very
convenient if you have any potential financial aids for me. I would
like to accompany my family, my wife and two sons.
Being
well aware of your outstanding academic accomplishments, I would
greatly appreciate if you provided me with an opportunity to work
as a research fellow under your supervision. I intend to return
to my present position in Korea after completing my fellowship.
I can start working from Sep. 1997 or anytime thereafter for 1 or
2 years under your circumstances. Enclosed is my curriculum vitae

Dear Dr. Jeong:
Dr. Copel has refered
your letter to me regarding the possibility of your visiting Yale
University. I have reviewed your CV and we would be happy to have
you come to our perinatal unit as a visiting professor and research
fellow.
I am currently an Assistant Professor at the Department
of OB/GYN. I am in charge of the perinatal research in the Division
of Maternal-Fetal Medicine. I am interested in the research of preeclampsia,
especially in establishing clinical and biochemical markers. These
markers include thrombomodulin, beta core fragment hCG etc. Human
umbilical vein endothelial cell and trophoblast are available in
my lab for in vitro study. I am also interested in investigating
the role of cytokines, chemokines (IL-8, Gro-alpha), nitric oxide,
and adhesive molecules (ICAM-a, E-selectin) in the pathogenesis
of intra-amniotic infection. We do have amniotic bank available
for the research in intra-amniotic infection.
As a visiting
professor and research fellow, you would have an opportunity to
observe the fetal echo, ultrasound, high risk patient care, and
certain procedures. In addition, you would have an opportunity to
do the researches in my lab. Currently I have one technician, two
postdoctoral fellows, one chief resident, and two medical students
in my research team. Again, we do welcome your visit, however, we
cannot offer financial support during your visit at Yale. If you

written to Dr. Hsu regarding visiting the Perinatal Unit at Yale.
However, I do need the following information from you as soon as
possible in order schedule your visit.
- Please state the desired dates of your visit.
I will do my best to accomodate your request, however there
is a limited amount of space available to visitors.
- Full name, date of birth, place of birth.
- How will you be paid while at Yale, personal
or by institution? Amount of money you will be bringing with
you.
- If married, will your family be visiting
with you? If so, I would need full names, dates of births and
places of birth for each member.
- Health insurance is required to enter the
U.S. and to visit this department.
- A current CV and what you would like to do
while at Yale is needed so that we can have one of our faculty
dictirs be a "sponsor". Note that the department cannot
subsidize you in any financial way.
- Please advise me as to your current VISA
status if known.
Once I have all the above information, I can then
send the necessary paperwork to Yale's Foreign Students and Scholars
Office. They will do the necessary paperwork for a VISA. This is
usually sent three months before you would leave the contry. Also
as soon as you send me the required information on housing and insurance.

IAP-66 도착
관용여권출국예정일 3개월 전에 IAP-66라는 VISA
신청서류, Family Attachment, 연수예정대학의 초청장이 도착하였다.
IAP-66 form과 Family Attachment는 필요한 곳이 여러군데이므로 미리
여러장을 복사해두는 것이 좋다. 또한 미국에 도착한 후에도 IAP-66
form과 Family Attachment는 여권안의 적당한 곳에 부착해 두는 것이
만일에 대비해 유용하다.
VISA 신청을 할 때 미국대사관으로 보내면
봉함을 하여 여권 안에 붙여두는데 그 이후에는 이 서류를 구경조차
하지 못한다. 실물 사진을 보이려고 했으나, 미리 scan을 하지 못해
나중으로 미루어야 할 판이다.
국외 파견 계획서
IAP-66 form이 도착하면 의과대학을 경유하여 대학본부에
국외파견에 필요한 서류절차를 신청하게 되는데 각각에 필요한 서식을
"아래 한글 3.0"으로 작성하여 다운로드할 수 있도록 링크하였다.
필요한 서류는

가족이
함께 갈 경우에는 "주민등록등본 1부"를 첨부하여야 한다.
5가지 서식을 하나의 파일로 만들어 올렸으니 아래의 "신청서식"을
클릭하여 다운로드 받으면 된다.

때, 파일이름 끝에 ".hwp"를 붙여주어야 한다.>
"파견근무기관의 요청(동의)서"는
IAP-66 form과 함께 연수기관에서 작성한 편지가 동봉되어 오니
그것을 복사하고, 번역하여 제출하면 된다.
"경비부담증명서"는
국가기관, 지방자치단체, 학술재단 등에서 지원받거나, 소속학회에
부탁하여 만들 수 있고, 아니면 사립의 장학재단 등을 통할 수도
있다. 가능하면 공공기관의 지원을 받으면 일이 깔끔하게 처리된다.
이상의 서류는 의과대학 행정실로 제출한다. 제출하면 의과대학
행정실이나 본부 교무과에서 어떤 여권으로 출국할 것인지를 문의하게
되는데 각자의 편의에 따라 정할 수 있다.
여권 신청
파견승인은 본부에서 결정하게 되는데 서류 제출후
약 3주 정도 지나면 국외파견 승인공문이 온다.
관용여권을 만들어
출국예정인 경우는 대학본부에 미리 알려주어야 한다.

여권발급신청서는 외무부나 도청에 가면 서식이
있고 즉석에서 적어낸다.
외무부 여권과는 종로구청 오른쪽
옆의 "제화재(?)보험 빌딩 4층"에 있다.
참고로 관용여권의
경우 본인은 5년, 가족은 1년으로 발급된다. VISA는 인터뷰 없이 아주
빨리 발급된다.

- 관용여권 신청전에 이미 일반여권을 소지하고
있는 경우
도청 혹은 외무부에 반납하여 무효처리하던지,
- 보관조치를 부탁하여야 여권발급신청이 접수된다.
- 이유는 한사람이 2개 이상의 여권을 동시에
소지하지 못하기 때문이다.
- 이미 보관 혹은 반납조치한 경우는 지참하지
않아도 단말기에 그 사실이
- 조회되기 때문에 신경쓰지 않아도 된다.
관용여권의 장단점

- 관용여권으로 일단 출국하면 출장명령을 받은
기간내에 임의로 귀국할 수 없으며,
- 혹시 귀국하였다고 하더라도 출입국관리소,
외무부 등에 신고하여야 하며,
- 대개 빠른 시간내에 다시 출국하여야 한다.
- 세관이나, 외국에서의 특혜도 없으며 가족의
경우 1년 만기의 여권이 나오므로
- 별로 장점이 없다는 것이 많은 사람의 의견이다
- 단, 외국에서 불의의 사고를 당했을 때 좀 빨리
처리된다는 것이 장점이라고 한다.
VISA 신청
여권이 발급되면 VISA를 신청한다. 빠를수록 좋다.
신청시 필요한 서류는 아래와 같다.

- 영문 재직증명서 (대학본부에 신청, 충북대학교의
경우 INTRANET 이용)
- 영문 갑근세 납부증명서 (의대 행정실에 신청)

VISA 신청은 여행사에 대행시킬
수 있다. 본인의 경우 여행사(성일항공,
02-736-3030, 김영식)에 의뢰했으며, 모든
서류를 영문으로 준비하여 보내면서 1인당 33,000원씩 보냈다. 번역이
필요한 경우 번역료를 추가하여야 한다. 대개 1인당 40,000원씩
받는 것 같다.
총장명의의 추천서는 대학본부에서 알려주었다.
미리 문구를 만들어 인쇄하여 교무처 교무과 연경흠 계장에게 가져가면
총장의 사인을 찍어준다. 아래에 그 서식을 참고하기 바란다.
주민등록등본의 번역본도 아래에 예시하였다.
VISA note는 직접
작성하던지 아니면 여행사에 의뢰하면 된다.
